**The short answer**

Group hospitalisation and surgical (GHS) insurance pays hospital and surgical bills for your employees under one yearly renewable policy. It is usually the core of an employer medical plan, with optional add-ons for outpatient visits, dental and maternity. It is not unlimited: there are waiting periods, exclusions and premiums that can change at renewal, so it pays to read the schedule of benefits before you buy.

## What does GHS usually cover?

GHS is about being admitted to hospital or having surgery. The exact limits sit in your schedule of benefits and vary by insurer, but cover typically includes:

- Hospital room and board, including intensive care
- Surgical fees, operating theatre charges and anaesthetist fees
- Hospital supplies, medicines and diagnostic tests linked to the admission
- Treatment before and after the hospital stay, if the policy includes it

Treat this list as a starting point, not a promise. Two GHS policies with the same name can have very different limits per day, per year and per illness.

## What are the optional add-ons?

Many insurers let you add riders on top of the base GHS cover:

- **Outpatient (clinical):** GP and specialist visits that do not need admission
- **Dental:** routine dental treatment, which base GHS normally excludes
- **Maternity:** delivery and related costs, usually with its own waiting period

GHS is also only one part of a group benefits package. Employers often pair it with group term life, personal accident, critical illness or hospital income cover. Our guide to [how group medical insurance works](https://www.coverbuddies.com/en/knowledge-hub/understanding-group-medical-insurance-in-malaysia) explains those components.

## What does GHS usually leave out?

Exclusions are where employees are most often surprised. Common examples include:

- Pre-existing conditions and specified illnesses that arise during the waiting period
- Cosmetic or plastic surgery, unless medically necessary
- Vision correction such as LASIK, and glasses or contact lenses
- Dental treatment, except for accidental injury
- External devices such as hearing aids
- Private nursing, rest cures, and injuries from illegal drugs or intoxication

The list in your own policy wording is the one that counts. Ask for it before you commit.

## How do waiting periods work?

A waiting period is the time after an employee becomes eligible when certain claims are not yet payable. One common structure applies a shorter wait of about 30 days for general illnesses and a longer wait of about 120 days for specified illnesses and pre-existing conditions. Your policy may differ.

Ask two things: whether new joiners serve the waiting period again, and whether your existing staff get credit for time served when you move from another insurer.

## Why can premiums change at renewal?

GHS is renewed every year, and premium rates are generally not guaranteed. Insurers look at your claims experience, changes in headcount and age mix, and medical cost inflation. A year with several large claims can push the next renewal up, so keep an eye on claims and talk to your broker before the renewal date, not after.

## Questions to ask before you buy

- What are the limits per day, per year and per illness?
- Is cover cashless at the hospitals your staff actually use?
- What are the waiting periods, and do they apply to new joiners?
- Are pre-existing conditions excluded for good, or only for a period?
- Which riders can I add, and what do they cost?
- Can directors, owners and dependants be included?
- How are renewal rates set, and what happens if headcount changes mid-year?

## Common questions

**Is GHS the same as a medical card?**
Not quite. A medical card is usually an individual policy. GHS is a group policy that covers a whole team under one contract, usually at a lower cost per head, but with less room to tailor cover for each person.

**Is group medical cover compulsory for employers?**
It is generally an employee benefit rather than a general legal requirement, although your employment contracts or an industry agreement may promise it. Foreign workers have their own rules: see [SOCSO, SPIKPA and workmen's compensation for foreign workers](https://www.coverbuddies.com/en/knowledge-hub/foreign-workers-in-malaysia-socso-spikpa-and-workmens-compensation-insurance-explained).

**How many employees do we need?**
Insurers usually set a minimum group size, often around four to six employees, though it varies. Smaller teams may need to start with standalone medical cards.

**Does GHS replace SOCSO?**
No. SOCSO is a national social security scheme with its own rules. GHS is a private policy. They do different jobs.
